Opt out form for any sanctioned X-League bowler. This form is to be filled out completely and submitted ONLY by the currently sanctioned Bowler themselves. This form is not to be submitted by anyone else nor by the team owners. If a team owner needs to remove any bowler they should complete and submit the "Update Roster Form".
