example: if John Smith is being transferred from one team to another, the new owner of the bowler should be filling out this form. previous owner should have already released bowler from previous team and do not need to submit this form.
Bowler leaving one team to join another must have a "Change Roster Form" filled out and submitted by the Current Team Owner prior to this form being filled out. They must also have paid their sanction fee. If they did not pay a sanction fee they must be added to a team roster and pay the sanction fee online. This form does not service for new bowlers/free agents.
